This form allows the candidate to describe how their teaching activities have influenced their own personal development as well as the development of their co-educators. Candidates must be reasonably new educators in the field of Health Professions Education, involved for under 10 years post-graduation. Any involvement in Health Professions Education before graduation is not a requirement. 

This form is in two parts:

Part 2 (Optional) - A Personal Letter for those who are self-nominating only.
